Required documents for applicants who have never been issued a social security number
Applicants born in the U.S. or Iran:
Applicants under age 12:
- Original valid U.S. passport of the applicant.
- Original valid U.S. passport of the signing parent
- Original U.S. Birth Certificate or CRBA (Consular Report of Birth Abroad) or Naturalization Certificate of the applicant.
- Form SS-5 "Application for a Social Security Card"
- Form ''Sworn Affidavit'' (only available at our office).
- Form 1706-f3 for U.S. born applicants only. (Available at our office).
- Original valid U.S. passport of the applicant
- Original valid U.S. passport of the signing parent
- Original U.S. Birth Certificate or CRBA (Consular Report of Birth Abroad) or Naturalization Certificate of the applicant.
- Form SS-5 "Application for a Social Security Card"
- Proofs of prolonged residency outside the U.S. (Original school transcripts of the applicant, starting from elementary school to present, officially translated in English, medical records, work documents etc.).
